Registration Paper Request Form

Congratulations on your new family member!

Please wait at least two weeks before you submit a request for your puppy's registration papers.

If you would like to personally pick up your puppy's registration papers directly from our store please be sure to include that in the comments. As soon as we process your request your puppy's registration papers will be waiting for you to pick up from the store. Please give us 7-10 days to process.

If you would like your puppy's registration papers mailed directly to you please provide your mailing address in the form provided below. Please double check your address before submitting it to us for any errors as we will not be responsible if your papers are mailed to the wrong address. Please give us 7-14 days to process your request and mail time. We are not responsible for mail that is lost through the USPS.

Please have your puppy contract ready as you will need it to complete this form. This form must be completely filled out in order for you to receive your papers. Once you receive your registration papers please keep it in a safe place. The original copy is given to you so there are no other copies available. Thank you.
